Evaluation of vaginal fluid β-HCG for the diagnosis of premature rupture of membranes

Abstract: Objectives: To determine the role of quntative level of β-HCG in vaginal fluid in diagnosis of premature rupture of membranes. Design: Case-control study. Setting: Department of Obstetrics and Gynecology, Al-Azhar University Hospitals. Population: A total of 150 consenting patients were evaluated, of whom 50 met the criteria for membranes rupture as a PROM group, 50 met suspected criteria for rupture membranes and 50 without rupture of membranes as a control group. Methods: quntative level of β-HCG in vaginal fluid. HCG is present in AF as well as maternal blood and urine, at concentrations ranging from approximately 2000 to 70,000 mIU/mL. Results: It was found that HCG levels were higher among PROM group compared to suspected and control group with a highly statistically significant difference between three groups. It was observed that vaginal HCG values more than 62mIU/ml are highly predictive of membranes rupture with 100% diagnostic sensitivity, 98% diagnostic specificity, 100% PPV and 98% NPV. The performance of this marker in diagnosing PROM was therefore very good. Conclusions: the presence of vaginal HCG is highly predictive of membrane rupture. Although a positive result would be strongly indicative of membranes rupture, a negative result would require further confirmatory tests to completely rule out membranes rupture.

Clinical Audit of Normal Labor at Al Azhar University Hospitals

Abstract: Background: Clinical auditing is a quality improvement process that involves reviewing the delivery of healthcare to ensure the best practice is being carried out. Objective: This work is aiming at auditing the current management of normal vaginal delivery at Al Azhar University Hospitals to improve emergency obstetric care. Methodology: This study has been undertaken in Al Azhar University Hospitals in the period between 1/7/2018 to 31/4/2019. This audit was conducted as cross sectional study that is planned to include 300 cases of normal deliveries. Policies and routines of normal labour and delivery practiced in maternity wards of Al Azhar University Hospitals was Compared with criteria and standards of ACOG guidelines. Results: During management of the first stage of labour, communications between the care-givers with women and her companions were missing in 89% of cases. Also, giving woman information and explanation was missing in 46.3% of cases. Using noninvasive and non-pharmacological methods of pain relieve during labour was missed in 99% of cases. bed rest after rupture of membrane was not done in 46% of cases. During management of the second stage of labour, woman, choice for position was not supported in 83% of cases. Also, continuous emotional and physical support to woman throughout labor in 19% of cases. Regarding episiotomy, skin was sutured continuously and no episiotomy suturing by vicryl in 100% of cases. Regarding management of third stage of labour, oxytocin was administered mostly by intravenous drip. Cord traction was mostly steady. During fourth stage of labour, close monitoring and surveillance of the woman at first 6 hours after labour was done. Hand washing before dealing with the baby was missed. Conclusion: That there are missing important aspects of appropriate care especially those related to appropriateness of care givers- patients interaction, safety and completeness of examinations, and appropriateness of postpartum care.

Abstract: The purpose of this paper is to examine the impact of organizational justice and Job Embeddedness through the mediating role of Trust in Evaluating Performance. This study was conducted with a sample of 280 faculty staff of Imam Abdul Rahman Bin Faisal University. Data were collected by administering a structured questionnaire and analyzed using structural equation modeling (Warp PLS 5). The results reveal that Trust in Evaluating Performance plays the role of a partial mediator between both procedural justice, informational justice and Job Embeddedness. The study was limited to four faculties represented by the colleges of Imam Abdul Rahman bin Faisal University, which restrict their circulation. This scholarly work may encourage managers and decision makers to develop trust in evaluating performance by focusing on organizational justice and job embeddence to flourish an environment of harmonious performance, further more to formulate effective strategies for cultivating facilitative work environment to enhance positive attitude among the employees to challenge future. The research is exclusive in determining the influence of organizational justice on job embeddedness through the mediator of Trust in evaluating performance in the new perspective of higher education that supplies empirical evidence to the extant literature.

Abstract: Growing interest in studying the mechanisms of governance and how to activate it after the financial crises and collapses of major international companies were the need to develop and implement mechanisms that are binding on all enterprises in order to maintain The rights of all stakeholders and to achieve effective oversight and accountability through integration and interaction between these mechanisms, the study aims to measure the effect of applying the internal mechanisms of corporate governance (Board of Directors Audit Committee) on internal audit and quality measured m in 3 dimensions (objectivity-professionalism-the nature of internal audit work) in the Saudi commercial banks and included Sample 3 commercial banks (the largest three commercial banks in Saudi Arabia in terms of assets), the survey was also distributed lists of employees of the internal audit department in the three banks to measure the variable The study's results, to confirm the existence of moral effect variable properties of Board and Audit Committee on audit quality, therefore the study recommended activating mechanisms of environmental governance, Saudi Arabia And to increase the independence of the internal audit Department of commercial banks in Saudi Arabia. Purpose: This study is designed to determine the effect of corporate governance mechanisms (Audit Committee-the Governing Council) on components of quality (objectivity-professionalism-the nature of the work) internal audit to be more effective and efficiency. Design/methodology: Applied study done by measuring governance variables field of financial reports issued by banks (Ahli-Al Rajhi-Saba) which represents a sample search, as the questionnaire lists were distributed The internal audit Department staff and members of the Audit Committee and Board members to measure the dependent variable (the internal audit quality). The results: There is a positive impact to corporate governance mechanisms applied moral quality internal audit. Value: his research provided practical guide on the importance of corporate governance mechanisms in the activation of the internal audit quality in commercial banks to be an internal sections that add value to this sector, especially with the growing Official interest in Saudi Arabia internal audit where Saudi society was established for internal auditors under the Cabinet of Ministers on 28/2/2011 in addition to novelty The subject of corporate governance and obliging companies by the CMA applying some articles list of governance. Abstract: Background and Purpose: Drug-drug interactions (DDIs) is frequent in critically ill patients due to polypharmacy, different drug groups and prolonged ICU or hospital stays. The aim of this work was to assess the risk factors for potential drug- drug interactions in surgical ICU patients, Zagazig University Hospitals. Study design: Cross-sectional study. Setting: Surgical ICUs (SICUs), Zagazig University Hospitals. Subjects: We included all patients admitted to SICUs, Zagazig University Hospital for 6 months period, as number of cases with this inclusion criterion at SICUs, ranged from 20-30 per month, so on our study of 6 month on record-based data took files of 120 cases. Methods: All patients were subjected to demographic data; age and gender. Clinical history data; main diagnosis, ICU stay in days, transfer from emergency department (ER) or from other departments, mechanical ventilation, previous surgeries, state of consciousness [Glascow coma scale (GCS)], Comorbidities. In hospital medication details: Total number of prescribed drugs in every day of ICU stay had been collected, number of different pharmacological and therapeutic subgroups prescribed and number of physicians who prescribed therapy. Interaction checker data (number and description of the DDI); the presence and classification of DDIs (for every day of the patients, treatment) had been determined by parallel use of two interaction checker data-base: Medscape and Lexi-Interact. Results: In the studied population, the age of the studied group ranged from 0.5 to 90 years with mean 43.74 years. Regarding sex 52.5% were male and 47.5% were female. According to Frequency of different Drug interactions among the studied group, 94.2% of the studied group had drug interactions. In Medscape checkers the most frequent interactions among studied group was minor (78.3%), then monitor closely (69.2%), serious (30%) and contraindicated (1.7%). The most frequent interaction in Lexi comp interactions drug checkers was C (monitor therapy) (75.8%), then D (43,3%), B (37.5%), X (8.3%) and A (5%). Total Medscape median 4 interactions per patient with range from 0 to 20. And total Lexi median was 3 interactions per patient with range from 0 to 17. There were +ve significant correlation between number of drug interactions and length of hospital stay, no. of drugs and no. of drugs groups among both Lexi-comp and Medscape interactions. Conclusion: To conclude, more than 94% of Patients in ICU usually have Drug drug interaction and this is related to increased number of drugs, drug groups in ICU and length of stay among both Lexi-interactions and Medscape drug checkers. The most frequent interaction in Lexi comp interactions drug checkers was C (monitor therapy) (75.8%) while in Medscape checkers the most frequent was minor (78.3%).

Standards for Choosing and Assessing Integrated Automatic Systems in Libraries and Information Centers: Applied Study on Egyptian Universities Libraries

Abstract: Information explosion with severe need for recent and accurate information with minimal time and effort – an objective for libraries and information centers – in addition to massive advances in computer systems, increased electronic expertise and skills (personally and professionally) and notable improvements in local and international communication networks are all reasons for the use of integrated automatic systems in libraries and information centers during the past few years. Since the late 1990s, these systems are heavily used in Egyptian libraries and information centers with the objective of developing information technology and validating files when limitations of information systems – imported with high cost – in the Kidney Hospital of Mansoura University appeared in automatic storing of information, data and registries. This led to the suggestion of establishing a technical center to fulfill the needs of scientific and information centers and associations for information systems instead of wasting money on buying only one system. This is followed by establishing the Center for Telecommunication Technology – Mansoura University. Eng. Mohamed Al-Refai designed the future system or "Egyptian Universities Libraries Consortium" (EULC) and he is responsible for it technically and an applicably (http://srv3.eulc.edu.eg/eulc/libraries). The system proved effective since its use in Mansoura University Libraries in 2005. The Egyptian Universities Libraries Consortium via the Supreme Council of Egyptian Universities, chose it to be applied in all Egyptian libraries and credited it as an efficient system for satisfying the needs of University Digital Libraries in addition to its relatively low cost. The consortium chose that system after evaluating all other local and international systems previously used in Egyptian University. It was proved that the system avoided all clear limitations of previous systems like "Advanced Library Information System" (A-LIS), "Library Information System" (LIS – LIS-2), "Horizon"… etc. The current research aims to assess the automated system used in the Library of Faculty of Physical Education for Women – Alexandria University, as the library uses Egyptian Universities Libraries Consortium (EULC) as all other Egyptian Universities Libraries since 2006 till now. Assessment is done according to standards for assessing integrated automated systems for libraries and information centers established by Dr. Osama Al-Sayed Mohamed Ali, assistant professor of libraries and information – Faculty of Arts – Cairo University. standards were concluded according to review of literature in addition to standards used in the "American Library Association" (ALA) and "Library of Congress" (LC) with modification to be fit for use according to the needs of Arab Libraries. Research tools included field visit to the library under investigation, application of the standards, review of related literature concerning assessment of integrated automated systems and interviewing system users including: Dr. Mohamed Al-Refai (system designer) – librarians – library administrators – specialists of digital libraries. Assessment depended on the availability of required standards in the system or the system's satisfaction for standards required by the library under investigation. Assessment included (17) standards each one of them included several items formed as questions with total number of (301) items. Through analyzing these items, the system was assessed and positive aspects and compliance with standards were discussed in addition to discussing the system's limitations and non-compliance with standards. Suggestion to avoid limitations were introduced.

Three Dimensional Assessment of soft tissue and Airway changes after Rapid Maxillary Expansion in Cleft Palate Patients using Cone Beam CT

Abstract: Objective: Measuring air way and soft tissue changes in cleft palate patients after rapid maxillary arch expansion by bonded hyrax appliance using cone-beam computed tomography. Material and methods: RME for 10 cleft patients ranged from 8-12 years old then pre and post expansion CBCT image was taken to them and analyzed by on demand 3D app software (Cybermed, South Korea) was utilized for linear and angular measurement. Results: Significant increase in nasal width, air way width, no change in air way depth and enhance soft tissue measurement. Conclusion: Enhanced air way and soft tissue measurements after RME on cleft patients.

Incidence of Neurosensory Alteration Post Articaine in Adult Patients at KAUFD

Abstract: Objectives: Local anesthetics are associated with minimal side effects and complications. Several retrospective studies proved that Articaine 4% and Prilocaine 4%, are associated with paresthesia compared to the other local anesthetic agents in dentistry. However, limited prospective studies reported similar findings. The present study is a prospective observational study evaluating and comparing incidence of neurosensory alteration after Articaine 4% local anesthesia and comparing the findings to Lidocaine and Mepivacaine local anesthetic drugs. Materials and Methods: Patients undergoing dental procedures receiving local anesthesia Articaine 4%, Mepivacaine 3% and Lidocaine 2% were followed up and evaluated for altered sensory sensation on the third and seventh post-operative days. Results: The results proved that 2% Lidocaine was found to be the most common type of local anesthesia used (37.4%), followed by 2% Mepivacaine (29.3%), 4% Articaine (25.3%), and 3% Mepivacaine (8.0%) with no reported sensory alteration in any case. Conclusion: Articaine Hydrochloride 4% was found to be safe and effective to use in different dental procedures without any associated sensory nerve alteration.

Evaluation of mandibular alveolar bone & tooth cementuminosteoporotic postmenopausal rat model

Abstract: Back ground: Osteoporosis that usually occur during postmenopausal stage is considered a progressive bone disorder. It was proposed that the principal cause of osteoporotic postmenopausal isestrogen deficiency. The present research was aimed to examine histologically the status of alveolar bone & tooth cementum in osteoporotic postmenopausal rat model. Material and methods: Twenty virgin female rats were split into two equal groups; ovariectomized (OVX) rats, Sham groups. After four weeks, the bone samples containing the molar teeth were collected from the euthanized animals and processed for light microscopic examination. Results: Histologic and histomorphometric analysis of alveolar bone samples revealed significant reduction intrabecular bone pattern and disorganized bone structure in OVX rats compared with sham rats. OVX rats showed more osteoclasts than sham rats. Loss of continuity and resorption areas of cementum were noticed in OVX rats. Conclusion: OVX induced osteoporosis can alter the microstructural of the tooth cementum and alveolar bone.

Improving the performance of a planar UWB antenna by adding cover layers

Abstract: In this paper, we investigate the effects of adding cover layers to the multi-layer structure of a planar UWB antenna on the antenna performance parameters. With the proposed technique, the antenna return loss can be enhanced and the matched frequency bandwidth can be increased. In some cases a multi-band antenna can achieve the performance of a wide band antenna covering all its original frequency bands and the gaps between them. With proper selection of the cover layer material, thickness, and position (on top, bottom and/or both), we can get significant improvements in the antenna return loss. In the same paper we investigate the effect of cover layer on other antenna performance parameters, such as gain, efficiency and radiation pattern. Using the solder mask as an antenna cover layer is a very useful technique that we propose. It is very economical, widely available and very easy and fast to fabricate.

Single Anastomosis Sleeve Ileal Bypass (SASI) for management of obese patients with type 2 Diabetes

Abstract: Background: Amidst the current worldwide epidemic of type 2 diabetes mellitus (T2DM), the global diabetes health burden is projected to reach 522 million in 2030, with much of this increase occurring in developing countries. Aim of the work: to evaluate the role of laparoscopic sleeve gastrectomy with loop bipartition (single anastomosis sleeve ileal bypass) as a bariatric and metabolic procedure in control of type 2 diabetes in obese patients over one year post-operative. Patients and methods: This prospective cohort study included 20 obese adult patients with type 2 DM recently diagnosed within last 5 years. Some of them have other associated comorbidities. They were recruited at department of surgery Ain Shams University. The follow up was obtained during the first year post-operative. Results: In this study, complete remission of diabetes was achieved in 75% of the patients by the 3rd post-operative month and in 95% by the end of the study. This was beside marked weight reduction and improvement of lipid profile without causing micronutrients deficiencies during the study period. Conclusion: SASI bypass can be one of the most efficient metabolic procedures and could be associated with less risks. The procedure should be considered under investigations until enough long term data are available. Thus it is worth to be explored in research aiming for more data.

The Negative Appendicectomy Rate in Different Age Groups Compared with Preoperative Investigations.

Abstract: Introduction: Despite being one of the commonest emergency surgical procedure, about 20% of appendicectomies performed world wide yields histologically normal appendix. This study compares the normal appendicectomy rate (NAR) in different age groups compared with preoperative investigations aiming to reduce NAR. Method: This study included 458 patients who underwent appendectomies at Ain Shams University Hospitals in Cairo and Queen Elizabeth Hospital – King’s Lynn, UK. All of them had emergency appendectomy between 01/06/2016-30/06/2018. The results were divided into three groups: Appendicitis, normal appendix, or other findings. Laboratory and radiological findings including white cell count (WCC), C-reactive protein (CRP) level, abdominal ultrasound (USS) and computed tomography (CT) scan results were collected and compared with histological findings. Results: We had 458 appendicectomies performed. The rates of inflamed, normal, and other findings were 69.2%, 16.4%, and 14,4% respectively. 96.2% of patients with appendicitis had raised WCC or CRP. Appendicitis was histological confirmed in 96.4% of patients with radiologically diagnosed appendicitis (97% and 93% for CT and USS respectively). Women of childbearing age had (30.6%) NAR compared to (9%) in men. Conclusion: Women in childbearing age have the highest NAR. Normal inflammatory markers and radiology support a more conservative approach in this group to reduce the overall NAR. Low-dose or reduced-range CT scans might be considered in this group.
